The mission of Sioux Rivers MHDS is to provide welcoming and individualized brain health and physical disability services to Iowans and their families in Sioux, Plymouth, Lyon, O’Brien, Emmet and Dickinson Counties. We are recovery focused, trauma informed and dedicated to providing transformative service for those experiencing a range of health challenges. Our services are divided into two categories available to all members of the communities we serve.

Resources

Regional Mental Health and Disability Services

Provides access, information and coordination of funding, while providing for the health, hope and successful outcomes of the residents in our region who have mental health and intellectual/developmental disabilities, including those with multi-occurring substance use issues, health issues, physical disabilities, brain injuries and other complex human service needs. Our services are provided through a network of regional partners.

Assessment and Stabilization Center

The Sioux Rivers Regional Assessment & Stabilization Center is two programs in one convenient location. The center is staffed by a team of professionals available 24/7 to individuals experiencing an increase in the severity of their mental health symptoms. This is a warm, inviting place for individuals to stabilize and return to the community. Our services in this area include:

  • Crisis screening and assessment
  • Access to medical personnel
  • Access to a brain health professional for consultations 24/7
  • Medication administration
  • Short-term therapeutic programming and/or skill-building in group or individual settings
  • Care coordination through stabilization plans, action plans and treatment summaries
  • Referrals to appropriate community resources
  • Discharge planning and follow-up services
