We are a collaboration of six Iowa counties: Clay, Osceola, Palo Alto, Kossuth, Winnebago and Worth. Our purpose is to have a coordinated system of care for citizens experiencing mental health conditions or intellectual disabilities.
We strive to improve health, hope and successful outcomes for the adults and children experiencing mental health or intellectual disabilities in our area. Care Connections of Northern Iowa was formed under Iowa Code 28 E to create a MHDS region in compliance with Iowa Code 331.390.
